1. Uninstall any previous installation of Adobe Reader.
3. Once the CD loads, automatic installation will begin. 4. In the “first tuesday Forms-on-CD Setup” window, click next. 5. Follow the onscreen instructions to complete installation.
Getting Started: 1. Double click the “Forms-on-CD” icon on your desktop. 2. Once the icon is clicked, the Forms-on-CD Welcome Page appears. From the Welcome Page you can access instructions on how to use the Forms-on-CD, form indices, reading materials and other useful features. 3. Links which can be followed appear in blue. Click on the link to follow it.
4. To return to the Welcome Page, click “Back To Welcome Page” on the top of the page. Forms-on-CD won’t install. 1. Remove CD from the CD-ROM drive and restart the computer. After restarting, reinsert CD. Installation should automatically begin. If it does not, see the Alternative Installation Instructions. Alternative Installation Instructions: 1. Insert CD into CD-ROM drive. 2. Right-click Start button. Select Explore. 3. Click the CD-ROM drive which is indicated by a picture of a CD above a CD-ROM drive. 4. Double click the icon labeled “Forms-on-CD.” Follow the onscreen instructions. 5. Once installation is complete, a “Forms-on-CD” icon will appear on your desktop. Mid-Installation Troubleshooting: Previous installation of Adobe Acrobat Reader prevents installation of Forms-on-CD. If you have a previous installation of Adobe Reader on your computer and have not uninstalled it prior to the installation of Forms-on-CD, the CD will not install properly and the following message will appear:

7. Click Finish. This will cancel the installation. However, you will now need to manually uninstall the programs that were partially installed prior to cancellation.
8. Click Start. 9. Select Settings. 10. Click Control Panel. 11. Double click Add or Remove Programs. 12. Uninstall both Adobe Reader and first tuesday Forms- on-CD by clicking on their name and icon then clicking the Remove button.

You can uninstall these by choosing the desired program from the list. 13. After both programs have been uninstalled, remove CD from the CD-ROM drive and restart the computer. After restarting, reinsert CD. Installation will automatically begin. Post-Installation Troubleshooting: No Welcome Page appears after launch. 1. If a blank CutePDF screen appears instead of the Welcome Page, close every currently open window until all are closed. 2. Click the Forms-on-CD icon on your desktop. The Welcome Page should now appear. 3. If a blank CutePDF screen appears again instead of the Welcome Page, click File in the upper left corner of the screen. 4. Click Open. 5. Click on the Hard-Disk icon which will be located in My Computer. 6. Click Program Files. 7. Click first tuesday Forms-on-CD. 8. Click Forms_CD_Welcome PDF file. The Welcome Page will now appear. |
